Investigation Summary
On December 31, 2002, Employee #1 was installing a ceiling sheet of drywall from an eight foot fiberglass ladder. He attempted to put a screw into the center of the drywall with his electric screw gun, lost balance and fell. He landed on a concrete floor, broken drywall around him. Employee #1 fractured both forearms at an odd angle, and was hospitalized.
