THE RAKE DRIVE ASSEMBLY WAS UNDER CONSIDERABLE TORQE, CAUSING TWO ANCHOR BOLTS TO SHEAR AND RELEASE THE DRIVE AND ASSEMBLY. EMPLOYEE #1 WAS HOSPITALIZED. THE ACCIDENT OCCURRED ON MARCH 20, 1987.
