Investigation Summary
At approximately 2:00 p.m. on March 24, 1999, Employee #1 was working as a laborer for Larson Roofing in San Francisco. The crew was tearing up the roof when it began to rain. As they were laying out plastic sheeting keep water out, Employee #1 stepped on a section of rotten plywood and fell approximately 20 ft to the ground. He sustained a fractured right wrist and arm. Emergency services was called, and he was transported to San Francisco General Hospital.
