Investigation Summary

Investigation Nr: 201177698
Event: 03/15/2005
Employee Is Injured in Fall from Fixed Ladder

On March 15, 2005, Employee #1 was working at a restaurant that was under construction. For some reason, he needed to gain access to the roof of the restaurant. He entered a fire suppression room and began climbing a fixed ladder towards a roof hatch that would allow him access to the roof. As Employee #1 reached for the top rung of the ladder with his left hand, which he was unsuccessful in doing so, his right hand released from the ladder, causing him to fall approximately 18 feet to the floor. Emergency services were contacted, and Employee #1 was taken to UC Davis Medical Center in Sacramento, California for evaluation. He was later transferred to Kaiser Medical Center for hospitalization and treated for a fractured vertebra at the T-12 joint. Employee #1 was released five days later.
