Investigation Summary

Investigation Nr: 201177136
Event: 02/28/2005
Employee's Arm Is Fractured in Paint Mixer

On February 28, 2005, an employee was using an industrial mixer called a dissolver. He was adding a 50 lb bag of ground limestone to a 55 gallon drum. The drum contained approximately 15 gallons of water and was located directly under the mixer with the mixer shaft and impeller lowered down in the drum. The employee had turned on the mixer and the impeller was moving while he was adding the limestone. While adding the limestone, the employee's loose cuffs on his coveralls became entangled in the moving shaft of the machinery. This force pulled the employee into the drum. The employee was hospitalized and treated for a fractured arm.
