Employee Is Injured in Fall from Elevated Cage

At about 4:30 p.m. on September 21, 2004, Employee #1, was working in the man basket elevated on a forklift. When the basket became detached from the fork lift mast, he fell approximately 10 to 12 ft onto some material. Employee #1 was hospitalized for fractured ribs and internal injuries. The investigation revealed that the man basket may have been improperly attached to the mast of the forklift. The accident could not be re-created using the method of attachment known to be used previously.
