Investigation Summary
At approximately 10:15 a.m. on March 28, 2011, Employee #1 was using a gas powered hedge trimmer to trim a hedge on the grounds of condominium complex. The complex was located over an underground parking structure with 15 skylights. He was working in proximity with an adjacent skylight. As he was trimming the hedges, he needed to reach the middle of a hedge. Employee #1 walked onto a skylight to trim it, but the skylight broke. He fell approximately 15 feet, striking a concrete floor and sustaining a back of the head laceration and an ankle fracture. Employee #1 was transported to a medical center, where he underwent surgery and received post-operative care, and was then hospitalized.
