Investigation Summary

Investigation Nr: 202542908
Event: 01/18/2011
Employee Suffers Finger Amputation

On January 18, 2011, Employee #1 and a coworker were adjusting a conveyer belt chain and sprocket drive system. The total distance between the sprockets was approximately ten inches. After they were finished and the guards were back in place Employee #1 thought maybe the chain was too tight so he reached under the guard just as the conveyor started. The chain caught his glove and pulled his finger into one of the sprockets. His right middle finger was injured and subsequently required amputation to the base of the fingernail.
