Investigation Summary
At 5:15 a.m. on September 13, 2010, Employee #1 collapsed at a Burger King parking lot located in Los Angeles, California. He was pronounced dead at 5:33 a.m. by Paramedics of an apparent heart attack. Employee #1 was a supervisor working for Andrews International. He worked the night shift from 11:00 p.m. to 7:00 a.m. According to company policy, he had two 10-minute breaks and one 30-minute meal break during his entire shift. His job duty included driving from place to place to check upon officers' status such as when they were taking breaks, time card, etc. Andrews International is engaged in security and risk mitigation business.
