Investigation Summary
At approximately 10:30 a.m. on April 26, Employee #1, a cable installer, was installing cable, running a line from the phone pole to the customer's home and grounding it. Employee #1 used a 28 foot fiberglass extension ladder on the roof of the home to reach the pole and fell off the ladder.
