Employee Seriously Injured Falling from Ladder

Employee #1 went to repair a roof leak in a student housing building. The building is a condominium and access to the roof is through a small room that has a 13 ft high job built ladder going to a roof hatch. As Employee #1 came down the ladder one side of one of the rungs came loose, and he fell approximately 12 ft to a concrete surface. Employee #1's injuries were complete spiral fractures of the left tibia and fibula. Employee #1 was hospitalized for three days.
