Investigation Summary
On June 24, 2010, Employee #1, an electrician, was performing his regularly assigned duties of working on power poles. As he used an extension ladder to reach the top of a temporary power pole that was 20 feet above grade, and measured 6 inches by 6 inches in cross section, it cracked and broke at the base. The pole began to fall, causing Employee #1 to fall 15 feet from the ladder to the ground. Employee #1 was taken to a local hospital, where he was hospitalized and treated for a laceration to his head. The incident investigation determined that the accident was caused by excessive force exerted on the temporary power pole by the extension ladder. The employer was cited for failure to report the serious accident, as well as for failure to establish/implement an Injury and Illness Prevention Program (IIPP) with adequate inspection procedures.
