Employee's Leg Is Fractured in Fall from Ladder

At approximately 1:00 p.m. on June 22, 2010, Employee #1, a maintenance mechanic, was performing a plumbing repair on a tank-less water heater located on a wall 11 ft above the shop floor. He was tightening a plumbing fitting while standing on a unsecured ladder, 5 ft high. Employee #1 was working alone at the time of the accident and had not received any training. He and the ladder fell, and Employee #1 was hospitalized with a fractured leg.
