Investigation Summary
At approximately 8:54 am, on June 3, 2010, Employee #1, a plastering employee of Samitaur Constructs fell from a scissor lift while doing stucco work on the side of a commercial building. Employee #1 and Employee #2, his assistant, set up the BARECAT scissor lift (model BC-2133-2W, serial HK 78213) at the shop about 100 feet away from the worksite. Employee #1 and Employee #2 took down the top rail at one side of the platform that was facing to the work surface. In order to take that top rail down, Employee #1 had to take the top rail of two end- rails down. After finishing the set up, Employee #1 drove the scissor lift from the shop to the worksite. According to Employee #2's statements, Employee #1 had worked on the scissor lift about two hours prior to his fall. Also according to Employee #1 statements, he worked on elevated platform (about 11 feet high) with no guardrails at one end of the platform. As the work was in progress, Employee #1 moved the scissor lift downward while he was on the platform. Employee #1 did not remember how the accident happened but woke up injured on the ground. There were no actual witnesses to the accident. Employee #1 was taken to UCLA Hospital, and then transferred to Kaiser Hospital. Employee #1 was hospitalized for 7 days total. As a result of the accident, Employee #1 suffered four broken ribs, broken left shoulder, and broken left pelvis.
