Investigation Summary
At approximately 2:00 p.m. on December 21, 2009, Employee #1 returned to his work station after his lunch. A coworker walked into the office and saw him sitting at his desk with his eyes and mouth wide open. The coworker call him, but he did not respond, and immediately called emergency medical services. Employee #1 was taken to Centinela Hospital and later that afternoon was pronounced dead at hospital, as a result of heart attack.
