Investigation Summary
At approximately 9:23 a.m. on September 9, 2009, Employee #1 was a construction worker. He was using a ladder and was pulling nails from an overhead beam. The work was part of a dry rot repair on the outside of a condominium complex, near a parking area. Another worker, who was not adequately trained, was operating a forklift. The forklift struck the ladder, which caused Employee #1 to fall and fracture his wrist. Employee #1's injury was treated at Little Company of Mary in Torrance.
