Employee'S Finger Is Amputated When Crushed By Forklift

On March 18, 2009, Employee #1 and a coworker were loading cargo into a container. The coworker was trying to fix a skid, 48-in. by 40-in. inside a container, 10.3 ft by 2.16 ft by 4.75 ft, and asked Employee #1 to guard the bottom of the skid where the blades were and make sure no holes punctured the bottom of the boxes he was loading. The coworker was using a Toyota Forklift, Serial Number 82545, and Model Number 7FGU25. It is equipped with side shift to position air cargo. Employee #1 was squatting beside the forklift to check the load, when the operator was loading a box into a container for air cargo transportation. Employee #1 put his hand on a shifting rail on the forklift. The coworker moved the shift and crushed Employee #1's fingers. Employee #1's left ring finger was amputated to the first knuckle and his pinky is under therapy after it was crushed and surgically put together. Employee #1 was hospitalized.
