Investigation Summary
On March 11, 2009, Employee #1 was removing a window, cutting the metal at the bottom of the window. The Makita 4.5-in. angle grinder he was using struck him in the face. He was wearing goggles but not a face shield. The grinder was not guarded and had the wrong size blade attached. He suffered lacerations to his nose, mouth, gum and cheek and was hospitalized.
