Investigation Summary
On February 13, 2009, an employee was unloading boxes from the bottom end of the conveyor that connected the upstairs stock room with the downstairs receiving area. Her finger was pulled into the conveyor belt and crushed. The employee was taken to Kaiser Permanente and released the next day after undergoing surgery, resulting in amputation of part of the crushed finger.
