Investigation Summary
At approximately 7:00 a.m. on January 9, 2009, Employee #1 was working for a roofing subcontractor in the renovation of an existing tilt-up warehouse. An aluminum extension ladder was set up on the mezzanine floor of the warehouse in order to access the roof. As Employee #1 ascended the ladder and reached the top of the ladder, it fell backwards, causing Employee #1 to slip. He attempted to grab the roof hatch, but was unable to reach it. Employee #1 fell about 12 feet, landing on the mezzanine floor with the ladder still in the vertical position. Employee #1 was taken to a nearby hospital, where he was hospitalized and treated for a fractured vertebrate at the L1 joint. The incident investigation attributed the accident to (1) the ladder not being secured at the top, and (2) a failure for the ladder to be angled with a 1 to 4 ratio.
