Investigation Summary

Investigation Nr: 202540217
Event: 01/05/2009
Employee Falls, Strikes Head, Requires Stitches

At approximately 9:15 a.m. on January 5, 2009, Employee #1 was working as part of a group that had finished cleaning up outside classrooms. The group (building and ground workers) split to check light bulbs in each classroom and office. Employee #1 went to room 118 to check light bulbs. A few hours later, Employee #1 walked toward the cafeteria and other workers saw dry blood on his head and shoulder. Other workers asked Employee #1 what happened. Employee #1 could not remember anything about the accident and why blood was on his head. The coworkers went to classroom 118 and saw a pool of dry blood on floor. There was nothing out of ordinary in the classroom except couple tables next to the blood were not straight. Employee #1 suffered a head laceration which required some stitches. Employee #1 was taken to the hospital and was released from the hospital after few hours.
