Investigation Summary
At approximately 7:00 a.m. on March 10, 2004, an employee was preparing to make a chalk line on a recently complete concrete block wall. He accessed the location on the wall by using a 24 foot fiberglass portable extension ladder. The ladder slipped out from underneath him and he fell approximately 15 feet to the concrete pad below. He was hospitalized with fractures to his left wrist and knee.
