Employee injured after falling from scaffold

At approximately 8:30 a.m. on March 2, 2004, an employee was erecting the second frame on a rolling scaffold in the rotunda. There were not enough locking pins. The employee used wire to secure the railing on one side. He did not properly secure one end of the mid-rail on the other side; he put the locking pin in backwards. The employee was down on his knees reaching for a wood plank being handed up to him by a coworker. The employee had his hand on the mid-rail; the rail failed and he fell to the floor. The employee suffered from multiple facial fractures and fractured right shoulder. He was hospitalized and treated for his injuries.
