Investigation Summary
At approximately 1:00 p.m. on February 6, 2004, Employee #1 was working for the Carpenter Company in Riverside, California. The employee was assisting a coworker, a machine operator, to clear a fiber web that had accumulated on a Garnett Machine's Incline Apron Conveyer. Both employees were working outside the fenced area. The coworker used a pole to move the fiber web off the running conveyer and pull it close to the fence where the employee could reach it. The fiber web became caught in the lower unguarded projecting shaft end of the conveyer. The employee had a hold of the fiber web when the rotating projecting shaft end twisted the fiber web like a rope very tightly around the employee's left hand. When he pulled his hand clear, the distal tip of his middle finger was pinched off. The Riverside City Fire Department paramedics responded to the accident and took the employee to the Riverside Community Hospital. Employee #1 was hospitalized, underwent surgery and was released the following day.
