Investigation Summary
At approximately 4:00 p.m. on January 25, 2004, Employee #1, with Madison Home, was standing on the platform of a 5-step portable metal ladder, 4 feet 3 inches high, removing boxes of wooden bed railings from a metal rack. The wooden railings slipped out of the box and struck his knee. Employee #1 then lost his balance and fell from the ladder platform onto the concrete floor. He was hospitalized at Moreno Valley Medical Center for a fractured jaw and left wrist.
