Investigation Summary
At approximately 3:30 p.m. on December 16, 2003, Employee #1 was working as a regular employee of the Fisher Ranch, which grows corn, wheat, hay, and cotton. He and a coworker were using a cotton module compactor. Employee #1's coworker was sitting at the controls and operating the equipment. Employee #1 was on the ground throwing loose cotton into the cotton module equipment. To clean the machinery, Employee #1 climbed onto the cotton module equipment using a metal ladder and reached over the two feet from where he was standing. As he was cleaning the machinery, the two pieces of metal that compact the cotton came together and caught his right hand, crushing it. Employee #1 was taken to Palo Verde Hospital in Blythe, CA, and was transferred to Good Samaritan Hospital in Phoenix, AZ. He required hospitalization for five days. Employee #1 had three surgeries, which included installing pins inside his right hand.
