Employee amputated three fingers in sprocket chain drive

On October 31, 2003, Employee #1 was checking the counters on the FMC Lemon Filler Machines to assure that the correct amount of lemons were going into the boxes that were being filled. The employee walked over to the unguarded sprocket chain drives to check the counters. While checking the counters, he observed a lemon that had dropped out of the box and was located between the sprocket chain drive that takes the boxes from the lemon filler machine to the rollers. Employee #1 attempted to remove the lemon and in doing so, his right hand came into contact with the moving unguarded sprocket chain drive. The employee amputated his right middle, index and ring fingers and went to a local hospital for treatment.
