Investigation Summary
At approximately 7:28 a.m. on October 18, 2003, Employee #1 and a coworker were attempting to start a FMC multi-fruit juicer, Model Number PO51, Serial Number 61020106. Employee #1 removed the back cover, or guarding component, of the juice maker, and was attempting to bump start the juicer drive motor. A coworker was standing in front of the juice maker with his finger on the start button, while Employee #1 attempted to manually manipulate the chain and sprocket drive assembly of the machine from the back. The machine was driven by a 12-inch sprocket with a 1-inch link steel drive chain. When the machine started, it caught Employee #1's right hand, resulting in the amputation of four fingers. He was transported to a local hospital in a privately owned vehicle, and then transferred to a larger facility, where he was treated and hospitalized for seven days.
