Employee amputates thumb in press brake

. On September 19, 2003, an employee was operating the large bending machine, Press Brake. He was bending metal material, measuring approximately 8-ft long, 5-in. wide and 0.125-in. thick. Operating the press brake involved pressing the right top button to level the piece of material being bent, pressing another top button to go up and down to adjust the material being bent, and then the foot pedal activated the bending process. The employee placed the material lengthwise and was adjusting the material with the two top buttons. The material was in place on the press's stationary bottom platform. The employee was holding, underhand, the material with his left gloved hand. The employee's left thumb was resting on top of the metal material being bent. He was adjusting the material to be bent with the two top buttons. The employee depressed the foot pedal with his right foot. He felt his left gloved hand being pulled into the machine as the bending process began. His left thumb was crushed during this process. The employee was transported to JFK Memorial Hospital and subsequently transferred to Loma Linda Medical Center. The employee was treated for amputation of left thumb to the first joint and was released.
