Employee's foot fractures when caught in stand-up lift

On July 10, 2003, Employee #1 was operating Crown Stand-up Lift Number 4. He was traveling from aisle 12 to aisle 26 to pick up a loaded pallet. He was traveling east and was turning right into aisle 26 when he took his left foot off the pedal to stop the lift. At the same time, he was steering the wheel to the right. Employee #1 could feel the lift start to vibrate when the stand-up forklift he was operating collided with a metal shield around a storage rack post. He did not recall his left foot leaving the area where the pedals were located. Employee #1's left foot was crushed between the metal shield around the storage rack post and the frame of the stand-up forklift. He sustained an open left calcaneus fracture and was hospitalized.
