Investigation Summary
At approximately 7:30 a.m. on April 15, 2003, Employee #1, a general laborer at a recreation vehicle manufacturing facility located at 13272 6th St., Chino, AZ, and other employees stacked eight, partially completed, recreation vehicle side-wall panels on a wheeled dolly. This dolly was to be pushed by a fork lift and manually guided and assisted by the employees onto a flat bed trailer for transportation to the next production station of the manufacturing process. Employee #1 was standing against the wall sections in an attempt to hold them in the upright position while the forklift and other employees moved the dolly. Employee #1 was killed when the load toppled over striking and trapping him under the walls. Each wall panel is estimated to weigh 583 lb. The wall sections measured approximately 438 in. by 96 in. by 2 in. and weighed approximately 583 lb each. The locally-manufactured wheeled dolly measured 37.5 in. long by 64 in. wide by 88 in. high and was constructed of 2-in. tubular steel.
