Investigation Summary
At approximately 6:15 p.m. on February 8, 2003, Employee #1 worked on a conveyor in Extruder-40, Line Number 4 in the Forming Department. He stacked the bird stops on the chain racks. He picked up the bird stops from the conveyor. He was walking back and forth. A bird stop became stuck between the two belts. As he pulled the jammed bird stop, his right middle finger became caught between the conveyor belt and the bird stop. His middle finger was amputated, and he was hospitalized.
