Employee injured while performing maintenance

At approximately 12:15 p.m. on March 8, 2003, Employee #1, a maintenance packing technician for a corn chip manufacturing facility, was performing maintenance on an Apex bagging machine (Make: Ishida Corporation; Model Number: 201). The maintenance being performed involved adjusting and servicing a moving flat belt. For some reason, Employee #1's left pinky finger became caught in the belt. He was hospitalized and treated for a severed pinky finger at the first digit.
