Investigation Summary
At approximately 11:12 a.m. on January 23, 2003, Employee #1 and a coworker were working in a steel fabrication plant. Employee #1 was primarily conducting his work from a shop, while the coworker was operating a forklift (Make: Hyster-210; Model Number: H210XR) to move trench shield panels from a press after they have been straightened. As the coworker was moving a 10-foot by 26-foot trench shield panel from a press, Employee #1 exited his shop and attempted to get the operators attention. The operator did not hear him and began to backup while Employee #1 was standing behind the forklift. For some reason, a dangling hook from an overhead crane became caught in a stacking eye from the shield, pushing it into a nearby I-beam. The I-beam fell over and landed on Employee #1's lower left leg. Employee #1 was hospitalized and treated for multiple fractures to his leg.
