Investigation Summary
On January 6, 2003, Employee #1 was conducting cleanup operations in a dirt field. A coworker was operating an Allis Chalmers forklift that was being used to move the cantilever metal storage rack. Employee #1 was being assisted on the ground with the preparation of relocating the cantilever metal storage rack to the pre-designated location near the employer's outer perimeter of the dirt field. The forklift operator reached the location where the rack was to be placed on the ground: the outer perimeter of the employer's property line located adjacent to the rear portion of the business establishment next door. Employee #1 stated he was working between the backside of the wall and the front of the cantilever rack. Employee #1 was placing the boards he had been carrying on the ground beneath the outer frame of the cantilever metal rack. Employee #1 stated that the cantilever metal rack on the forks suddenly shift and he attempted to move out of the way. However, he was struck on the right side of his lower body by the cantilever metal storage rack. Employee #1 was knocked to the ground. He was hospitalized for his injuries.
