Employee died after fall from ladder

At approximately 8:20 a.m. on December 17, 2002, Employee #1 placed a 14-foot Werner Job Master Aluminum Ladder, Model Number D1240-2, on top of the roof of a carport to gain access to the roof of the adjoining apartment to check the gutters. The carport roof was made of loose gravel and it was 10 feet from the carport roof to the apartment roof. The employee was approximately half way up the ladder when it slipped. He fell 7 feet onto the carport roof. He was transported to Arrowhead Regional Medical Center where he was treated and released. He returned on December 24, 2002 to Arrowhead Regional Medical Center complaining of pain and was diagnosed with a ruptured spleen and was hospitalized until he died on January 16, 2003.
