Investigation Summary
At 9:30 a.m. on November 16, 2002, Employee #1 suffered a serious injury with third-degree burns and broken tissues. The employer runs a distribution center with 350 non-unionized employees on the site. Employee #1 was a regular employee of the employer. At the time of the accident Employee #1 was operating a conveyor, a Kosan Crisplant Tilt Tray Sorter, Model TTMD-750. Employee #1 was watching the conveyor system. Just before the accident, a box, about 12-inches by 12-inches by 12-inches, got stuck or jammed on the conveyor near the idle roller between the two conveyor belts. As Employee #1 tried to free the box, his right hand got caught between the incoming belt and the idle roller which caused a serious injury to his right hand. Employee #1 was hospitalized.
