Investigation Summary
At approximately 10:30 a.m. on November 1, 2002, Employee #1 was operating an unloaded Clark forklift, Model GPX-20, Serial Number 0056-8360K for a manufacturer of planting soil and compost manure. While operating the forklift on soft soil with the forks raised 12 feet above the ground, the forklift tipped over and the employee was crushed under the cage of the forklift as he tried to jump out. The employee was killed.
