Employee amputates fingers after contacting saw blade

At approximately 8:15 a.m. on October 14, 2002, Employee #1 was working for Skyline Madmill Homes, in the mill department at table saw # 4. The employee was moving a material cart in front of his saw. He tripped over a 3-ft by 5-ft floor mat in front of his Rockwell / Delta 12-inch to 14-inch, 220 volt, table saw. The employee reached for the saw, attempting to stop his fall. When doing so, his hand slid across the saw table top and inadvertently came in contact with a damaged hood guard and the rotating 12-inch blade. Two fingers on his right hand were amputated. Employee #1 was hospitalized.
