Investigation Summary
At 11:30 a.m. on August 26, 2002, Employee #1 was part of a crew that was stripping the asphalt roofing from a field barn that was being demolished. Employee #1 was pushing an asphalt-encrusted, 4 ft by 8 ft sheet of plywood off the roof when he lost his balance and fell 20 ft to the ground, landing on his head. Emergency responders were called and he was transported by the Mentone Fire Department to Loma Linda Hospital, where he was admitted for treatment. Employee #1 died as a result of his injuries. There was no personal fall protection system or other means provided or used to protect employees from falling off the roof.
