Investigation Summary
At approximately 1:30 a.m. on June 24, 2002, Employee #1 was wiping moisture from the top of a slide mold opening of a "MAUSER, Model Number BM100" blow mold machine. The mold machine was in automatic cycle mode and was not guarded at the point of operation. The mold machine was not de-energized, stopped or mechanically blocked to prevent inadvertent movement of the mold. While Employee #1 was trying to wipe the moisture, the machine cycled and caught his left hand causing the amputation of his left index, middle & ring fingers. Employee #1 was hospitalized.
