Employee's finger amputated in belt and pulley

At about 8:30 a.m. on June 12, 2002, Employee #1 was cleaning a belt on a bottom coater (Belvac Production Machinery, Model BV-86) on line number 3 in the printing area. He opened the guard on the belt and pulley drive and was using a rag to clean the belt while the coater was running at idle speed (300 rpm). When the rag became caught in the pulley, Employee #1's right pinkie finger was pulled into the pinch point. He was hospitalized for amputation of the finger.
