Investigation Summary
At 7:46 a.m. on March 27, 2002, Employee #1, a regular employee of Pacer Technology, was operating a Pacer Model B2 semi-automatic glue filler machine. The employer is primarily engaged in manufacturing, packaging and distribution of adhesives, sealants and related products for consumer markets. Employee #1 was picking up glue-filled 10 cubic centimeter bottles from the discharge end with her right hand when her right index finger got caught between the rotating filler machine turntable and the bottle ejector bar, causing amputation of her right index finger. The contact point between the moving machine's turntable and ejector bar created a crushing or pinching hazard which was not guarded. Employee #1 was taken to the Loma Linda University Medical Center in Loma Linda, California for treatment and was treated and released the same day. A surgery was performed.
