Investigation Summary
At approximately 1:30 p.m. on March 15, 2002, Employee #1, a maintenance technician, was in the process of relocating electrical conduit and pneumatic lines along an elevated steel girder when the accident occurred. Employee #1 sustained serious head injuries as a result of a fall from a platform elevated by a forklift. Employee #1 was transported by paramedics to the Arrowhead Regional Medical Center in Colton where he was treated and hospitalized for 27 days and was subsequently transferred to a local rehabilitation hospital.
