Investigation Summary
At approximately 8:45 a.m. on February 7, 2002, Employee #1 was operating a Dayton Rogers, model number USAF368000 beader machine. He was in the process of placing a motor cycle exhaust tip, size 2.75-in. diameter, when he inadvertently activated the beader machine. At this time his left index finger got caught between the descending holding chuck and the moving post causing amputation of his left index finger. Employee #1 was taken to Arrowhead Regional Medical Center, Colton, CA. for treatment and was treated released the same day. A surgery was performed at a later date. The moving post and the descending chuck created crushing hazard and was not guarded at the point of operation.
