Investigation Summary
At approximately 7:45 a.m. on January 30, 2002, Employee #1, an assembler with Universal Forest Products in Riverside, CA, was operating a truss press machine. When she reached into the danger zone to reposition a nail plate on a truss, the machine cycled, and her right second and middle fingers were crushed. Employee #1 was hospitalized at the Orthopedic Medical Group of Riverside, Inc., with amputation of the two fingers.
