Investigation Summary
At approximately 8:40 a.m. on November 10, 2000, Employee #1, a production worker, sustained a traumatic partial amputation of two fingers and crushing of a third finger on the left hand as the result of an incident involving a 75-Ton Baldwin Defiance tablet press. Employee #1 was transported to Chino Valley Medical Center in Chino, California and was hospitalized for eight days. The incident occurred in the Chemical Production Room in the employer's facility, located in Ontario, California.
