Investigation Summary
On January 23, 2002, Employee #1, an experienced plumber working for a subcontractor, was installing overhead pipes using a new 12-foot A-Frame ladder. The pipes were being installed during a new construction project to build a city maintenance facility. Employee #1 slipped and fell off the ladder, fracturing his skull and two ribs. Employee #1 was hospitalized to treat his fractures.
