Investigation Summary
Employee #1 was breaking seals on drums that were stacked on a pallet. The pallet was staged on a 10-foot-high platform. The employee lost his balance and fell to the concrete floor, striking his head. Employee #1 died from his injuries two days later. The open edge of the platform was not guarded by a railing or chain at the time of the accident.
