Employee Suffers Amputation

At approximately 1:20 p.m. on March 14, 2001, Employee #1 was threading a paper medium on a corrugating machine. Employee #1 was standing on a stepladder to reach into the necessary part of the machine. Employee #1 pulled out the necessary length of the paper and handed the end of the paper to another employee on the other side of the machine. Then Employee #1 came down from the stepladder holding onto the frame of the machine when a previously threaded paper broke and a roller swung back onto his hand and crushed the tip of his left middle finger. Employee #1 suffered the amputation of the finger and was hospitalized.
